New Patient Forms
We have made our new patient forms available to you as Adobe Acrobat files. So that we can better serve you quickly, we recommend printing and completing these forms at your convenience before your appointment.
Please remember to complete and bring all pages with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.

On Fridays, our administrative team will be available from 8:00 am – 12:00 pm to assist you.
During your appointment, you have our undivided attention. Our entire team is focused on getting to know each patient and providing the highest quality dental care. Simply call our office to schedule an appointment, or take advantage of our convenient online appointment request feature. We will do our best to set appointments at your earliest convenience. If you need to cancel or reschedule an appointment, call us as soon as possible so a broken-appointment fee is not assessed.
After-Hours Emergency Care
We recognize that emergency situations arise and we will do our best to respond promptly. If you are an existing patient with an emergency and we are out of the office, please do not hesitate to call us at (501) 455-6100. You should leave us a voicemail message with your name, phone, number and the nature of your dental emergency, then press # and 2 to urgently deliver it to our pager. One of our team members will be paged and will call you back as soon as possible.
Insurance & Billing
We are committed to provide excellent care and ensure that you receive the treatment and services that best meet your needs. We will file most insurance plans, and we are in-network providers for Arkansas Blue Cross/Blue Shield, Delta Dental Premier and Metlife insurance.
Please call our office to check on your insurance availability. We do not participate in Health Management Organizations (HMOs) or Medicare/Medicaid programs.
During your visit, we sit down with you to discuss dental concerns, treatment recommendations and payment options. Our dedicated financial coordinator will file your insurance paperwork and help you find the payment method that’s right for you.
For our patients with dental insurance, we will estimate the your portion as best we can with what little information is given to us by your insurance company. You are responsible for all expenses not covered by your insurance, and those payments are due at the time of service. Payment arrangements must be made with our financial coordinator before treatment begins.

Payment Policy & Financing Options
Payment concerns should never be an obstacle to receiving excellent dental care. We accept cash, personal check and major credit card for payment in our office as well as CareCredit, a financing program that allow you to pay your balance over time in increments that fit your budget. And remember – our staff is available for all of your financing questions, so please don’t hesitate to call.
For many patients, CareCredit is an easy way to arrange long-term payment options with a great interest rate. You can even visit their website to be pre-approved for any level of dental or medical care. Call our office for details or stop by for an informative brochure.

Location and Directions
We are conveniently located on Stagecoach Road (Hwy 5) in Otter Creek, just minutes away from West Little Rock, downtown Little Rock, Bryant and Benton.
From Little Rock & West Little Rock
Take I-430 to exit #1 (Stagecoach Road). Off the exit ramp, turn west onto Stagecoach Road, going towards Bryant and Benton. Continue on Stagecoach Road for a little over 1.3 miles and we are located on the left, past Grampa’s Catfish and the Otter Creek Assembly of God Church.
From Bryant and Benton & areas South of Little Rock
Simply take I-30 to exit 128 (Otter Creek Rd.) turning north onto Otter Creek Boulevard. Turn right at the intersection of Otter Creek Boulevard and Stagecoach Road (Hwy 5). Continue on Stagecoach Road for a little over 1/2 mile and we are located on the right.
